A quick reference for the core Monarch Scribe workflows — recording a visit, uploading audio, managing visits and patients, and turning a transcript into a clinical note.

Welcome

Monarch Scribe is a medical transcription workspace: capture a patient encounter as audio, get a transcript, and generate an editable SOAP-style clinical note from it. Everything below assumes you are signed in — your visits, patients, and notes are private to your account.

Getting started

  • Sign in with your account. After signing in you land on Workspace — your home base, which surfaces in-progress visits, drafts awaiting review, and recent patients.
  • Use the left sidebar (desktop) or the bottom tab bar (mobile) to move between Workspace, Visits, Patients, Live Transcription, and Analytics.

Record a visit

Start a live recording from any of these:

  • The Start visit button at the top of the sidebar.
  • The Start patient visit action on the Workspace, or the Live Transcription page.

While recording:

  • Transcript text appears live as you speak. Pause keeps the session resumable; Stop ends it and marks the visit ready for review.
  • Each visit is auto-named (e.g. “Afternoon visit · May 25”). Click the name to rename it in place at any time.
  • If the connection drops, the session auto-pauses and saves — tap Resume to continue. You won’t lose what was already captured.

Upload a recording

Already have an audio file? You can transcribe it instead of recording live:

  • Upload as new visit — on the Visits list or the live controls. Creates a new visit and transcribes the file.
  • Attach a recording — on a visit’s Audio tab. Adds the audio to an existing visit and appends the new transcript below a labeled divider.
  • You can also drag & drop an audio file onto these surfaces.

Manage visits

  • The Visits list is searchable and filterable by status (Drafts, Awaiting review, In progress, Locked) and grouped by day.
  • A paused or draft visit shows a Resume action that drops you back into recording where you left off.
  • Lock a visit (from its menu) to prevent accidental deletion. Locking doesn’t make the note read-only — it only blocks deletion until you unlock.

Patients

  • The Patients page is your roster — search, sort, and open a patient to see their demographics and visit history.
  • A visit starts unassigned. Assign it to a patient from the visit’s actions (or the patient picker) so it shows up in that patient’s history.

Clinical notes

Open a visit and go to the Clinical Note tab:

  • Generate builds a draft SOAP note from the transcript.
  • Edit any section directly. Use Improve on a section to have AI refine it while preserving the clinical facts.
  • Copy note copies the full note (with section headers) for pasting into your EHR or another document.
  • Every edit and AI step is versioned, so you can review the history of a note.

Tips & shortcuts

  • Press ⌘K (or Ctrl K on Windows/Linux) to jump to any visit, patient, page, or action.
  • Switch the theme (Light / Dark / System) from the toggle in the top header on desktop, or from the menu under your avatar on mobile.
